Physician Dual Practice: Access Enhancement or Demand Inducement?

چکیده

In many developing countries, the majority of physicians employed in government clinics also have a private practice. We develop a simple model to show that allowing dual practice helps low-income governments retain skilled physicians to assure patient access. If dual-practice providers di¤erentially refer higherincome patients to private practice, public funding becomes more e¤ectively targeted on the poor. Yet dual practice physicians may also skimp on e¤ort, pilfer supplies, and induce demand. Patterns of care-seeking in Indonesia, especially disproportionate use of private providers by the urban poor, are consistent with exacerbated incentive for physician self-referral to private practice in urban areas. Supplier inducement in a public health care system.

The present paper examines whether supplier inducement exists for primary physicians' services in Norway. Our model assumes optimization behavior of the physician. The research design is adapted to the institutional setting of Norwegian primary physician services, where there is a fixed fee schedule. According to our model, the physician has three choices: to rational services, to supply an opt...

A model of physician behaviour with demand inducement.

We present a model of the physician-patient relationship extending on the model by Farley [Farley, P.J., 1986. Theories of the price and quantity of physician services. Second, we derive predictions from this...

Physician-induced demand for childbirths.

A controversial technique for testing the hypothesis that physicians induce demand involves two stage least squares (TSLS) regression analysis of cross-section data on physician supply and utilization. This paper tests the power of TSLS by applying it where there is at most only a trivial amount of demand inducement--the demand for childbirths. We find 'evidence' of inducement of childbirths, c...

Physician pricing and health insurance reimbursement

This study was based on physician claims records from three Blue Shield Plans. The principal results are: • Physicians are income-motivated. This means that income incentives can be incorporated into reimbursement systems to achieve specific ends, and that care should be taken to assure that new mechanisms do not create income incentives for physicians to act contrary to policy objectives. • Ph...
